Herts Young Homeless

Who: Herts Young Homeless is an independent registered charity, established in 1998, that helps young people secure and maintain appropriate accommodation by providing information, support, and help in a crisis. Herts Young Homeless works with other agencies throughout Hertfordshire towards improving local services and preventing homelessness.

How: We provided Herts Young Homeless with Salesforce support and the implementation of a “live chat” function to reduce the impact on volunteers and improve the service delivery to the vulnerable.

North Tees NHS Trust

Who:

North Tees and Hartlepool NHS Foundation Trust (NT&H NHS Trust) is an NHS trust providing healthcare services in the North East of England. It serves the areas of Stockton-on-Tees, Hartlepool, and parts of County Durham. The trust operates hospitals including the University Hospital of North Tees in Stockton-on-Tees and the University Hospital of Hartlepool in Hartlepool. As an NHS foundation trust, it operates independently but still provides services within the framework of the National Health Service in England. The trust is responsible for delivering a range of healthcare services to the local community, including medical, surgical, and emergency care, as well as specialized services in areas such as maternity, pediatrics, and oncology.

How?

Cognition24 supported North Tees in implementing Salesforce Sales Cloud, managing stakeholders, gathering requirements, configuring Salesforce, and creating custom fields. Stand By Me Bereavement Support

Who:

Stand-by-me Bereavement Support offers support and advice to children, young people and their families in North Hertfordshire and Stevenage. They offer a therapeutic group work programme for children, young people and their parent/carers. Their Contact-me model of support for Schools, provides a framework to support those bereaved in childhood throughout their school career and our Uniting Families project offers support to individual families that have been bereaved.

How:

Stand By Me, a non-profit organisation, was in search of a user-friendly cloud solution to meet various organisational needs, particularly focused on CRM functionality. They needed a system that not only provided clear organisational benefits but also ensured scalability and future-proofing. To meet these demands, Cognition24 carried out a Salesforce implementation customised to their specific criteria.

GRIT

Who:

Growing Resilience in Teens (GRIT) supports young people to improve their mental health through group boxing programmesworkshops and one-to-one coaching. Young people come to GRIT from a variety of routes, including health care professionals, education settings and self- referrals – the majority of referrals come directly from schools. At the core of their work is the ‘Four Corners’ method, which empowers young people to use adversity as a platform for growth.

How:

At Cognition24, we partnered with GRIT to implement Salesforce Sales Nonprofit Cloud, helping to streamline their operations. Through meticulous customisation and collaboration, we optimised staff, coaches and patient coordination, as well as the onboarding process. Our focus on data security ensured compliance while enabling seamless accessibility. This solution allowed the charity to strength their impact and propel their mission forward with renewed efficiency and confidence, reaffirming our commitment to leveraging technology for social good.

Herts Urgent Care

Who:

HUC, a social enterprise, provides high quality Urgent Healthcare and GP Services to over 3.5 million people across a range of counties, including Hertfordshire, Cambridgeshire, Peterborough, Bedfordshire, and West Essex. The Integrated Urgent Care model championed by HUC includes both Out of Hours and NHS 111 services as well as access to a Clinical Advisory Service.

How:

HUC wanted a user-friendly solution that delivered on several business requirements based around CRM functionality. The system needed to add value and provide clear business benefits. It was also crucial that the system was scalable and future-proof. Cognition24 conducted a Salesforce implementation based on this criteria, as an NHS provider HUC was not looking for a ‘Sales’ system, their primary focus for their CRM was to support with stakeholder management, as well as somewhere to keep information about tenders and potential areas for development.

St Mungo’s

Who:

St Mungo’s outreach teams go out each night to meet people who are homeless and to help them off the streets. With 17 outreach teams they are one of the largest providers of outreach services in the country. They believe that people can – and do – recover from the issues that cause homelessness.

How:

Working alongside Real systems, we provided custom functionality and enhancements to Salesforce not-for-profit.
